Customizable Fan Duct for All Metal Hot End and 40mm fan

The purpose of this adapter is to replace the noisy 30mm fan on my J-Head V6 Hotend with a 40mm Noctua fan.
The diameter of the heat sink (and some other parameters) can be adjusted with the Customizer.
